Responsibilities:
- Responsible for end-to-end evaluation of the multi-year project, including but not limited to:
- Design: Develop a common and feasible evaluation framework to measure project success, outcome, impact, cost/benefit and effectiveness in collaboration with project partners that align with committed project success indicators and best practices in social prescribing.
- Planning: Develop methodologies, processes, tools and materials tailored to the needs of each phase to support data collection.
- Monitoring: Ongoing support and mentorship for each phase to address barriers and support best practices in data collection and evaluation. Identify risks impacting evaluation and develop mitigation strategies.
- Evaluation: Conduct focus groups, surveys and interviews as required, support data collection and ensuring data are reliable and valid.
- Analysis: Collate, clean, prepare and analyze data, prepare accessible verbal and written reports in a variety of formats suitable for diverse stakeholders.
- Knowledge Mobilization: Support project management with evidence to inform continuous quality improvement, and present evaluation findings to diverse multidisciplinary audiences including project partners and CRC leadership teams.
- Thorough desk/secondary research and regular scans to maintain current knowledge of the social determinants of health, population health, and social prescribing with specific focus on financial assistance.
- Engage collaboratively and cooperatively with internal and external stakeholders, including participants, partners, decision-makers, researchers and funders.
