Description:

The Perinatal Pelvic Health Service (PPHS) Specialist Midwife will work with the PPHS Specialist Physiotherapist, Obstetric/Urogynaecology Leads for PPHS, the Directors of Midwifery and Local Maternity and Neonatal Services (LMNS). The role has been designed to support the transformation of maternity and physiotherapy care via the Perinatal Pelvic Health Project, to implement key elements of pelvic health innovation, care and transformation of services within the maternity setting across the Herefordshire & Worcestershire LMNS (Local Maternity & Neonatal Service). This will be delivered through a system wide approach across the Herefordshire & Worcestershire LMNS delivered through innovative projects, service re-design, education and training for staff and setting up Peer support and mentoring. The post holder will work with the PPHS Specialist Physiotherapist and Obstetric/Urogynaecology Leads for PPHS to undertake planning, co-ordinating, delivering and evaluating the service provided to patients with pelvic health conditions on a day-to-day basis, and work closely with and support other maternity teams working with pelvic health conditions throughout the Trust / Herefordshire & Worcestershire LMNS & ICS (Integrated Care System) ensuring effective cross boundary working which may include travelling to sites for the delivery of care within the Herefordshire and Worcestershire ICS.

KEY RESPONSIBILITIES Professional/Clinical Leadership The aim of this role is to improve the standard of the pelvic health service across Herefordshire & Worcestershire. To provide professional visible leadership across all aspects of perinatal pelvic health. Support the Clinical Lead in improving services in line with National policies and where appropriate, take a lead role on the midwifery issues related to the implementation of national guidance applicable to sphere of responsibility To support and work with Teams across Maternity to develop a programme to facilitate the implementation of clinically based teaching to support better care in women with obstetric pelvic floor injury. The post holder will facilitate a programme aimed at equipping both midwives and obstetricians to increase their knowledge and competence in identifying women with obstetric pelvic floor injury, creating bespoke pelvic patient care plans and physiotherapist referrals.
To develop and expand midwives competence in undertaking to promote education within maternity services to continue to improve intrapartum and postpartum perineal and bladder care and to support further reduction in rates of OASI Provide endoanal ultrasound scans for women with OASI and other non-pregnancy related bowel complications, as appropriate, providing feedback, recommendations and assurance to the patient and linking with other health care professionals as required. Provide and assist with the management of care for women with pregnancy related bladder problems. To continue to improve standards and quality of care in perineal and bladder care To work within an agreed job plan to meet service needs. To conduct holistic assessment and providing supervision and guidance to colleagues in skilled history taking, physical examination, observation and ordering, conducting and interpreting relevant tests and investigations.
To work in collaboration with the multi professional team, contributing expert, evidence-based knowledge and skills to relevant aspects of the patients care and treatment To address patients risk factors and health promotion needs through comprehensive assessment of individuals for risk factors To build an environment of trust and rapport, enabling patient empowerment in their healthcare and acting as an advocate for the patient in treatment decisions and choices. To draw upon an appropriate range of multi-agency and inter-professional resources to ensure that patients receive appropriate services, making referrals to other professions and services as appropriate. To use effective communication and interpersonal skills to provide information, explanation and reassurance to patients. To maintain accurate and contemporaneous records of patients assessment, care and treatment.
To implement and provide specialist evidence-based care for women with perineal trauma and pregnancy related bladder/bowel dysfunction, including the provision of endoanal ultrasound scans and counselling of results through the OASI one-stop service. To improve governance, patient safety and continuity of care in women with obstetric pelvic floor injury To promote education within maternity services to continue to improve intrapartum and postpartum perineal and bladder care and to support further reduction in rates of OASI. To work with our midwifery team in the development of a safety culture, in partnership with the matron for maternity services, consultant midwife, the education team, the clinical midwifery managers, obstetricians and develop creative new ways of learning. Provide on a regular basis both constructive and positive feedback and identify and disseminate learning from good outcomes.
To work in collaboration with, and support the delivery suite co-ordinators, the senior midwives on the wards, community, and MLU, and obstetricians, to ensure safe birth outcomes. Act as a role model demonstrating clinical leadership and midwifery expertise. Ensure a high standard of compassionate clinical care and record keeping in accordance with Nursing and Midwifery Council, national legislation, and local standards. To monitor the standards of care and take appropriate actions when standards fall below expected levels.
Continually evaluate and audit the practice of self and others at individual and systems levels, selecting and applying valid and reliable approaches and methods which are appropriate to needs and context, and acting on the findings Support the clinical Lead in ensuring that there is optimal utilisation of resources through effective use of physical and/or financial resources Staff Development Develop own and others knowledge and practice across professional and organisational boundaries. Ensure that midwives are aware of care pathways for women To ensure that a training and development plan, based on need, is produced and reviewed annually Abide by the The Code: Professional Standards of Practice and Behaviour for Nurses and Midwives (2015). Facilitate the development of other midwives within the area of responsibility ensuring appropriate succession planning arrangements are in place To support induction packs and orientation programmes that are in place for all new staff Strategic/Development Support the clinical Lead to ensure continuous development and improvement of perinatal pelvic health services Advising and contributing to Directorate service development plans, in order to promote new initiatives and plan Support the clinical lead or participate in the development of corporate and Directorate objectives/projects ensuring that the nursing resources available are targeted towards their achievement.
