Description:

As a team we put patients and their families at the heart of everything we deliver and the Band 5 Specialist Diabetes Nurse Development role will be an exciting opportunity to excel your carrier to a Band 6 DSN.
Gloucestershire Hospitals NHS Foundation Trust offers a countywide Diabetes and Endocrinology service in a variety of clinic-based locations. We are looking for a compassionate Nurses to join our team to help deliver high standard of care and work well within a multidisciplinary team.
You will be working with Band 6/7 diabetes specialist nurses who will be supporting you in achieving the competencies. You should also be able to evidence consistently developing your knowledge in order to provide patients with an advanced treatment options.
You should communicate effectively and actively participate in Multi-Disciplinary Team (MDT) meetings to aid patient care, joint treatment sessions, case conferences and specialist interest group meetings.
To establish a timely completion of diabetes specialist skills and competencies in a timely manner during your agreed development period. This will help to encourage patient self-managing, maximize rehabilitation potential and to ensure understanding of medication, diabetes as a life-long diagnosis, long term management of their condition, blood glucose & possible complications.
Support to manage caseload of countywide outpatients through clinic, including the administrative activities including dictating letters, organizing patient’s follow-ups. Also, to use clinical reasoning skills and techniques to assess, diagnose, plan and offer a range of treatment options; deliver, evaluate and amend the treatment plan, to make recommendations for appropriate and timely management in an outpatient and inpatient setting. Supporting admission to and discharge/transfer from hospital.
Work closely with the specialist Nurses and Lead nurse to ensure the operational delivery of high-quality care, clinical safety & effective clinical governance, ensuring appropriate cross county cover for diabetes nursing care within given resources. Also, to adhere to all Trust policies and procedures and professional code of conduct.
