Description:

We have an opportunity for a Band 6 Nurse to join the community team for learning disabilities.
We provide help and support for adults aged 18 and over who have a diagnosis of a learning disability.
We work as part of a wider multi-disciplinary team which includes nurses, physiotherapists, occupational therapists, speech and language, psychology, family therapy and psychiatry.
You will be required to hold a case of more complex clients and will liaise with all other professionals involved in the care of the individual.
You will supervise and support junior members of the team and will also be responsible for mentoring the nursing students on placement with the team.
Due to the geographical area covered the post holder must be able to commute in a timely manner in response to service need.
You will be required to complete initial contact assessments or learning disability screening assessments for those entering the service. As a caseload holder you will complete all necessary nursing assessments which will lead to care plans and risk assessments for all involved in the individual’s care to follow.
You will ensure that all junior staff are supported on a day to day basis and that all nursing students with the team have a daily timetable of activity. You will be required to attend the referral allocation meetings (RAM) to discuss new referrals to the service and to seek any further information that is required.
The post holder will assume the responsibility for the team in the absence of the Band 7 Nursing Lead . This will include dealing with day to day management of the team and also dealing with crisis cases that occur.
