Description:

This is an exciting opportunity to join the CNWL NHS sexual health services in the London Borough of Barnet as a specialist sexual health nurse supporting people with learning disabilities (PWLD) into sexual health as part of “The Bridge Service”. The
Bridge Service is a dedicated clinic for PWLD in Edgware and Vale Drive CNWL sexual health clinics.

Responsibilities:

  • Improving access and bridging any gaps for PWLD to sexual health advice,

information and the full range of clinical services.

  • Providing free, confidential and supported access to the Barnet sexual health

clinics for PWLD as well as working in the community, supporting family

carers and support workers and other involved professionals.

  • Providing reasonable adjustments to support patients such as longer

consultations, easy-read and accessible information.

  • Supporting health, social and VCS networks to identify PWLD and improve

engagement and planning care.

  • Community outreach sessions providing relationships and sex education,

asymptomatic STI screening in community learning disability settings,

including colleges and day services.

  • Establishing seamless pathways between sexual health services and

statutory, VCS and primary care services.

  • Providing consultations to assess sexual health needs, consent, safeguarding

and concerns around capacity; make reasonable service adjustments and

referrals as appropriate.

  • Embedding health promotion advice and supporting ‘Making Every Contact

Count’ towards reducing health inequalities.

  • Provision of LARC and integrated sessions within the main Barnet sexual

health clinics.

  • A full driving licence with access to a car is essential

Health Promotion and Disease Prevention (Public Health).
Health Promotion and Disease Prevention / Public Health when in Clinical Practice.
Proactively develops, implements and monitors public health and health promotion activities, including own clinical work, work of team and the local community in accordance with local and national plans.
To participate in clinical and educational outreach and participate in health promotion activity, contributing to the development of new services.
Participates in and influences local health policy development, including health impact assessments.
Engages and forms links with relevant stakeholders and professional agencies, providing expert training and advice where appropriate.
Identifies the needs of diverse/vulnerable groups, i.e. young people and contribute to the development of tailored services to meet their needs.
Undertakes relevant need assessment profiles.
Promotes safer sex and provides sexual health information to clients as appropriate.
Keeps up to date with and prevents cross infection through application of appropriate techniques.
Management of / provision of care for quality standards
Works unsupervised in the assessment, implementation and evaluation of client care
