Our ethos is to facilitate a comprehensive solution focussed offer for young people with mental health needs across the trust. We provide a senior clinician led consultation & advice clinical model to CWP teams supporting young people aged 17 to 25. We support the existing multi-disciplinary team in formulating an understanding of the young person’s biopsychosocial needs, supporting decision making and planning, and helping to identify pathways for ongoing care. Our key focus is to support teams working with young people with high complexity and where specialist consultation & advice from the 18-25 team is indicated.
We work in partnership with the following trust care groups: Adult’s Specialist Mental Health, Children, Young People and Families, and Learning Disabilities and Neuro-developmental disorder. This means the successful candidate will gain invaluable experience supporting young people with a wide range of complex diagnostic presentations.

The successful candidate will;

  • support in delivering a high quality, trust wide mental health consultation and advice model to teams that access the 18-25 service
  • take a holistic and solution focussed view of young people’s needs, whilst considering their strengths and aspirations.
  • support with the navigation through complex health, social care and educational systems, helping to reducing barriers to accessing services.
  • Liaise with a broad range of teams across the trust and associated partners such as ICBs
  • triage referrals and then utilising expertise within subsequent consultations (alongside other members of the team).
  • work autonomously yet collaboratively within a multi-disciplinary framework, providing and contributing to specialist assessment, formulation and clinical support.
