JOB SUMMARY

An exciting opportunity has arisen for a part-time Band 6 Respiratory Nurse to join our team in Southeast Hampshire on a year’s fix term contract. The successful candidate will work within an established team, you will have a passion for chronic disease management and committed to improving patient outcomes.
The service provides oxygen assessment and review, management of complex patients, early supported discharge, and admission avoidance.
The successful candidate will have experience in respiratory care and an enthusiasm and willingness to gain expertise within this field. Experience of oxygen assessment and community care would be desirable but candidates from community, secondary and primary care will be considered.

JOB DESCRIPTION

Responsibilities:

To work as an Autonomous Practitioner within the respiratory team, accepting referrals, providing comprehensive expert assessment and delivering care to individuals and groups of patients with respiratory conditions.
To provide a service within localities across Fareham, Gosport and SE Hampshire covering the home oxygen assessment and review service, rapid response to avoid admission and supported hospital discharge. Candidates will also be involved in delivering education to other professional and staff across the locality. They will work closely with primary care, respiratory consultants, community teams and other specialist services.
