Under the supervision and direction of the consultant, the post holder will be responsible for: Role in assessment of new patient referrals Care plan and treatment formulation, guidance on evidence-based treatment and effectiveness. Physical risk assessments and checking blood results. Liaison with paediatricians and acute medicine for patients requiring physical stabilisation at an acute hospital Ordering blood tests and ECGS Routine medication reviews Complex case management Consultation to the wider team Liaison and collaborative working with other services/agencies. Attendance at Team Business Meeting and Clinical Team Meetings Multi-disciplinary, multi-agency and partnership working Mental Health Act assessment, Mental Capacity Act Assessment
