JOB DESCRIPTION

• Obtains history, gathers triage information, assesses and evaluates patient health concerns made via phone, fax, email or other modes of communications to
• determine right siting of care and provide health advice according to established guidelines and protocols
• Develops, implements, and evaluates a plan of care for each caller’s presenting
• symptoms, Collects and analyzes data to establish health priorities and interventions.
• Streamline the triage process and shorten the waiting time for patients requiring Emergent care or Specialist consult.
• Maintains and safeguards confidentiality of patient health information and medical records.
• Communicates complex triage case with the relevant clinician.
• Network with the clinicians from clinical departments to establish and update clinical triage protocols,
• Provide daily updates and monthly reports of the Triage services.
• Provide regular in-house training and assist in preparing quiz for the department so as to improve the team’s medical knowledge.
