Description:
WORK REQUIRES EXPERIENCE AND KNOWLEDGE IN THE FOLLOWING:
- Bachelor’s Degree required
- Commitment to serving homeless individuals
- Minimum of 2 years’ experience in providing case management
- Experience in program management and staff supervision
- General knowledge of local community resources
- Proficient in Microsoft Office
- Good verbal and communication skills
- Ability to work independently
- Ability to multitask, maintain confidentiality, and meet deadlines
- Ability to maintain professional conduct, attitude, and appearance at all time
- Ability to identify underlying challenges and barriers
- Ability to have attention to detail
- This position requires a Valid Driver’s license with a good driving record
- Must be a proactive critical thinker and problem solver
Responsibilities:
- Have a working knowledge of all aspects of the SSVF Program Guidelines and CFCS SSVF Policies and Procedures
- Supervise and direct case managers to ensures that veterans receive a high quality of care
- Maintain their own caseload as needed
- Manage the day-to-day operations of the case managers in our catchment areas (Passaic, Morris, Sussex, Union, Hudson, Bergen, and Essex Counties)
- Review and approve all SSVF intake files for eligibility
- Ensure that clients receive appropriate services as required by our SSVF contract and SSVF Program Guidelines
- Follow Housing first approach to housing
- Review and approve all financial assistance requests submitted by case managers before submitting to the SSVF Director for final approval. In addition, ensure all requests are accurate and timely.
- Participate in all VA calls and trainings as assigned.
- Provide monthly statistical information
- Conduct bi-weekly supervisions with case managers
- Ensure eligibility screenings, intakes, contacts, housing stability plans, temporary financial assistance, and other information is documented timely and accurately in the electronic health record (AWARDS)
- Function as a team member by covering co-worker’s responsibilities, participating in program activities, and communicating relevant information to other team members.
- Develop and maintains relationships with local social services, public benefit agencies, shelters, faith based and community-based organizations.
- Collaborate with the Continuum of Care (COC) in our catchment areas and other VA, and community providers to ensure services are delivered efficiently.
- Actively participate in the Veteran By- Name List efforts in collaboration with the COCs and other VA and community providers in our catchment areas.
- Attend and participate in all staff meetings, trainings, and case conferences, as assigned
- Ensure staff are documenting service delivery accurately and timely while ensuring veteran confidentiality
- Communicate with staff and clients to resolve issues that may arise
- Conduct chart reviews
- Ensure all client files contain all required documentation as per the SSVF Guidelines
- Have a working knowledge of AWARDS/HMIS database
- Monitor all screens to ensure contact is made within 24 hours of receipt of screen
- Ensure all data is entered accurately and timely into the AWARDS database
- Transport clients in agency vehicles in a safe, cautious, and responsible manner
- Communicate all barriers and/or issues to SSVF Director
- Submit monthly reports as required
- Adhere to agency policy, procedures, and professional code of ethics
- Responsible for other duties that the SSVF Director may assign from time to time to support the effective and efficient operation of Catholic Charities/SSVF.
