Hours: 22.5 hours per week Salary: £24,245.10 per annum (pro rata) Holidays: 25 days per year plus 8 public holidays (pro rata) Period of Notice: 1 week during probation; 4 weeks thereafter Probationary Period: 6 Months Based at: Neath Port Talbot Responsible to: Carmarthenshire County Manager Role Purpose: To deliver services in the community to substance users aged 18 years and over to help reduce the harm caused by their substance use. This includes individuals who have not accessed treatment and those who have repeatedly not attended appointments with alcohol and drug treatment providers, and have complex and multiple needs such as: Alcohol and/or drug use Mental ill health Homelessness, rough sleeping and vulnerably housed Anti-social behaviour and offending behaviour The post holder will support the Engagement Outreach Worker to carry out assessments, develop and review care plans, and liaise with partner agencies to ensure service users needs are met. The role involves supporting the delivery of a comprehensive support package; which may include advice and information, advocacy, needle exchange, health promotion, structured individual support, signposting and referral to treatment services, and access to wrap-around services. The aim is to empower service users to build their recovery capital, and support them into social (re)integration.
Service Delivery Responsibility for managing a team of staff who provide a range of interventions to individuals who require support for their drug and/or alcohol use. The Senior Service Manager will work as a key member of the team in developing and delivering the service in line with Adferiads values and develop close working relationships with external providers. The post holder will ensure that all staff are working to the statutory requirements of the service specification in line with the organisations policies and procedures. To shape the service to meet the needs of the community we serve. Main Duties and Responsibilities: Deliver an outreach provision in the local area, including crisis intervention and brief interventions. Undertake substance use assessments and develop, deliver and review care plans ensuring service users are central and directly involved in this process. Conduct risk assessments and develop risk management plans.
Understand the need to protect children and vulnerable adults, and adhere to all principles, policies and procedures in effective safeguarding. Work collaboratively with service users to help them identify and understand how their substance use may negatively impact on the lives of themselves, their families and the wider community. Carry a caseload and ensure continuity of care for service users through effective case management. Working and liaison with partnership agencies, both statutory, private and third sector.
Promote health awareness to individuals accessing the service and work in partnership with dedicated agencies to provide holistic support and assistance. Facilitate the Needle and Syringe Programme provision, providing appropriate safer injecting, wound care advice and overdose awareness advice, and Naloxone training. Offer and provide Dry Blood Spot Testing to those at risk of Blood Borne Viruses. Attend regular consultation and multi-agency meetings to address specific individual needs, to discuss referrals and progress.
Participate in organisational meetings, regular supervision sessions and annual appraisal. Take responsibility for updating training needs and knowledge requirements such as relevant legislation, and practice/research relevant to the role. Provide advice, support and training to colleagues in partnership agencies as necessary. Maintain appropriate written and database client records, and undertake appropriate administrative tasks in a timely manner.
Contribute to monitoring and reviewing procedures. Encourage service users to participate in service development and actively seek feedback on the service from stakeholders. Ensure quarterly statistics and Key Performance Indicators are achieved. Maintain confidentiality in accordance with Adferiads policy and procedure, and with any statutory/contractual requirements.
Work in a non-judgmental, anti-discriminatory and inclusive way at all times. Willing to work flexible hours to respond to the needs of the service. Carry out all tasks with professionalism and in line with the agencies policies and procedures, in particular Lone Working. Promote a positive profile of the agency.
