Description:
REQUIRED QUALIFICATIONS:
- Bachelor’s Degree in Nursing or
- Master’s Degree in Social Services or equivalent educ/experience
- Upon hire: Oregon Registered Nurse License
- 3 years Discharge planning and case management in an acute care setting
- Working knowledge of MS Office Suite, including but not limited to Word, Outlook, and Excel
PREFERRED QUALIFICATIONS:
- Master’s Degree in Nursing or Community Health
- Supervisor or lead experience
- Crucial Accountability Training
- Emotional Intelligence Training
