Description:
Job Title: Support Care Coordinator - South West Region
Location: Homebased with travel to multiple locations within the South West region
Salary: £16,224 - £17,881 (based on 3 days per week and depending on experience )
Hours: 22.5 (e.g. 3 days per week 9 - 5 with half hour unpaid break)

Walking With The Wounded (WWTW) supports those who have served in the Armed Forces, empowering them to regain their independence, thrive and contribute in our communities. WWTW recognises that for some ex-service men and women the route to care looks complex and often inaccessible. By providing an integrated care coordination service working alongside the NHS as part of Op Courage, WWTW will ensure these individuals are provided the breadth of care they need, as well as providing support to their families. The Care Coordinator will be an impactful and crucial service to a small cohort, which is highly in need.
WWTWs Care Coordination Service involves the assessment of needs for your client, the sourcing of appropriate support and the ongoing navigation of that support. You remain the central point of contact as you support your client to address their challenges, which can include a lack of secure housing, debt, social isolation, relationship breakdowns, lack of education and training, welfare issues or substance misuse.
We are looking for a Support Care Coordinator who fully appreciates the patience, resilience and self-motivation required to support a dispersed and diverse caseload of complex individuals. You must be able to work independently, particularly in managing your time and caseload, whilst recognising that you are one crucial element of a wider team of support who will need to work collaboratively to get the best outcome for clients. Experience of working with veterans would be beneficial, although not essential.
The Support Care Coordinator will have a caseload of complex ex-servicemen and women. Working with local delivery partners, you will make referrals for specialist support, whilst retaining the overall caseworker responsibilities. Your role will be crucial to the success of the service users outcome. If you are an experienced caseworker with an understanding of working with more complex clients, we would like to hear from you.
