Our Nova Program is seeking a full time candidate for the role of Support Liaison.

WHO WE ARE

Mountain Plains Community Services Society of the North (MPCSSN) is a non-profit, charitable society governed by a volunteer Board of Directors. Our Society has operated numerous diverse community and family-focused programs designed to address the unique needs of the Northwest region of Alberta since 1972. Currently, we offer services in partnership with Alberta Health Services and Alberta Human Services.

Responsibilities:

  • Use creativity and flexibility, to create a daily schedule based on program and individual needs, focusing on time for appropriate group activities and individualized programming.
  • Monitors and tracks individual’s person-centered plans, and works with program team to ensure deadlines are adhered to and recorded.
  • Assists clients in creating a network of community supports and involves relevant agencies and family members when applicable.
  • Coordinates and communicates all support strategies and community networking plans with the program manager, including monthly reporting on scheduled activities and their therapeutic value to persons served.
  • One weekend a month designated on-call for home program.
  • Monitors monthly progress reports and ensures appropriate filing of relevant documentation; communicating relevant information for quarterly and annual reports.
  • Completing Nominal Roll/ Occupancy reporting on behalf of their home program
  • Championing programming initiatives to other staff to ensure all staff are supporting clients through trauma informed, therapeutic interventions.
  • Participating in CARF meetings and ensuring CARF requirements are met in program.
  • Working with other Support Liaison’s to develop and implement agency wide therapeutic programming.
  • Attends any meetings, appointments, case-conferences and training as delegated by the program manager.
  • Participates in monthly supervisions with program manager and attends monthly staff meetings.
  • Liaising and collaborating with family members or natural supports of persons served to ensure a holistic approach to service delivery.
  • Supporting employee onboarding process within the home program.
  • Completes all other delegated tasks at the discretion of the program manager.
