JOIN US AS A SUPPORT SERVICE CO-ORDINATOR AT THIRTEEN GROUP!

We are looking for a Support Service Coordinator for 37 hours a week to cover maternity leave until March 2025 on our young parent supported housing scheme in Durham which supports young families that need support to be able to live more independently.

Responsibilities:

ROLE OVERVIEW:

Thirteen Group is on the lookout for a Support Service Co-ordinator to join our Care and Support Team. School View has nine flats along with a range of communal facilities, and you’ll be based in the office right at the heart of this scheme.

KEY RESPONSIBILITIES:

  • Run activities linked to seasonal events or celebrations or activities that help improve social isolation and specific learning linked to support. We’ll give you the creative freedom to provide positive outcomes for our customers.
  • Deal with any referrals received and assessing suitability for the service.
  • Work with individuals to create person centred support plans and hold weekly keywork sessions to achieve their goals identified.
  • Manage risk and safeguarding for all residents.
  • Attend multi agency meetings and provide written reports.
  • Manage void properties and allocations.
  • Responsible for managing building compliance and health & safety of the scheme.
  • Assist individuals to maximise their income and apply for relevant welfare benefits and grants.
  • Working as a team of four, you’ll work across a two week rota, covering our operating hours of 9am to 5pm on weekdays. You’ll be required to work one in two Saturdays , and the weekend you work, you will have the Monday after off. You’ll be expected to be on call one in every four weeks.
  • You’ll also have the opportunity to pick up additional hours on a relief basis, working across other schemes to gain insight into other parts of the business and create strong working relationships across teams.
