Description:

The Transformation Officer will be an important member of the team supporting the Community and Out of Hospital workstreams will provide support to the delivery of a portfolio of programmes focusing on supporting community-based provision e.g. community providers, hospices/EOLC, care homes and care providers, Examples of work programmes include – Enhanced Health in Care Homes programme, neuro rehabilitation services, and developing the community model of care.
They will provide day to day operational support to these programmes, leading on an agreed set of workstreams. They will provide support to SWL Head of Community and Out of Hospital Transformation – Community Services the to ensure the overall objectives of the team are met.
Priority will be given to internal SWL ICB staff who are currently “at risk” of redundancy.
- Supporting the planning and overall programme management of specific workstreams focused on improving patient flow. They will support the delivery of these programmes which are co-ordinated across all parts of the ICS working with partners such as local authorities, health and care providers, the VCSE and patients and carers.
- Ensuring that patient flow transformation plans they are supporting are evidence based and take a population health approach to understanding need, addressing health inequalities, and setting clear outcomes and standards to measure success. Support the use of population health management data is used in operational delivery as appropriate.
- Working in partnership with colleagues across Southwest London or Pan London to ensure alignment with other programmes. creating joint working to benefit of the whole of SWL.
- Supporting public and patient engagement to support the programmes. Support activities to ensure that the voice of people with lived experience / experts by experience are at the heart of all transformation programme activities associated with the portfolio.
- Working with the Place leads the postholder supports the co-ordination of system wide opportunities for change and transformation across programmes of work focused on improving patient flow.

Responsibilities:
FURTHER DUTIES OF THE ROLE INCLUDE:
- Liaise with external centres of excellence as necessary to ensure SWL is receiving best practice guidance to inform SWL delivery and transformation.
- Identifying where delivery of programmes in their portfolio is off-track, or where desired outcomes are not being achieved, and to propose and seek agreement to remedial action to address this, acting where necessary.
- Support the development of any SWL bids for community/out of hospital services transformation funding. Working with partners to ensure that SWL is in the best place to secure the maximise the opportunity any transformation funding offers. Working with the SWL Finance Team to ensure that any community/out of hospital services transformation money is appropriately accounted for and reported on.
- To keep up to date with national and local developments and research best practice models of care within Community/Out of Hospital services, in order to regularly provide expert advice to colleagues, partners and stakeholders, ensuring this knowledge is appropriately reflected in service planning.
- Be responsible for proposing and drafting changes, implementation, and interpretation to policies, guidelines and contracts which may impact the service.
- The postholder will have responsibility for supporting bids and relevant procurements in the most effective and efficient way.
- The postholder will be responsible for the management of the SBS (or equivalent system) raising POs, managing invoices under direction of the 8c and supporting the 8c to manage financial flows on the SBS system
