The Job Summary:
The LTC Projects Coordinator will provide support to the development and implementation of various projects within the Long-Term Care (LTC) Department. This role is vital in the planning, execution and evaluation of initiatives that will positively affect the performance of the LTC Department and enhance our service to LTC residents. This position requires strong organizational skills, attention to detail, and the ability to work collaboratively with various partners to ensure projects are completed in accordance with SJHCG’s mission, vision, values, and policies.

Key Accountabilities & Success Criteria:

  • Manages all project activities, issues, change requests, and risks related to all LTC initiatives.
  • Lead and support LTC project kick-off meetings, status meetings, issue meetings, lessons learned sessions, and project close-out meetings as required, and completed any required reporting.
  • Oversees the day-to-day activities of ongoing projects and translates strategic program objectives into operational activities.
  • Acts as a liaison with other departments and ensures that the work of the department is coordinated with various parts of the organization.
  • Monitors project performance while advising and consulting on target goals and performance issues.
  • Supports decision-making and recommends solutions prior to escalation to the Director and VP.
  • Proactively communicates and collaborates with internal departments and clients to keep them engaged and informed on project status and requirements.
  • Plans and oversees the preparation and dissemination of all project communications.
  • Researches and implements existing and emerging best practices and applies these insights to continually advance processes and programs.
  • Ensures the project deliverable/outcomes for existing and new initiatives are successfully executed and then transitioned to operational partners with a sustainability plan when required.
  • Implements effective measurement tools that will measure the impact of interventions and evaluate the return on investment and informs future initiatives/improvements and/or adjust initiatives, as appropriate.
  • Provides project management support to the development and implementation of strategic and operational initiatives.
  • Ensures proper tracking of improvement initiatives and project management documentation.
  • Creates reports and communication documents to report on program and initiative status.
  • Manages multiple projects and priorities at the same time with competing deadlines.

Qualifications:

  • Diploma or Bachelor’s degree in business, health profession or related discipline.
  • Three (3) plus years’ experience in project management or change leadership, preferably in the healthcare or long-term care industry.

Skills & Abilities:

  • Knowledge of the regulatory requirements in the Fixing Long Term Care Act, 2021 is an asset.
  • Knowledge of the MDS RAI 2.0 or MDS InterRAI systems.
  • Working knowledge of Point Click Care documentation operating system.
  • Experience with performance measurement methods.
  • Demonstrated strong competency in MS Office Suite.
  • Highly effective interpersonal, oral and written communication skills.
  • Attention to detail and proven prioritization skills with ability to manage multiple concurrent assignments.
  • Demonstrated experience in leading change and program management.
  • Collaborative team player able to build effective working relationships with colleagues, senior leaders, and partners in order to move initiatives forward.
  • Demonstrated experience with developing performance and evaluation indicators and subsequently measuring performance against the indicators.
  • Ability to work independently, manage multiple tasks and deadlines, and solve challenging problems in a fast-paced, environment while also working collaboratively in a team environment.
