Description:

We are looking for a Band 6 nurse to join one of our Community Nursing teams based in ABC PCN (Maidstone), to support the development of this service. The service operates seven days a week from 08:00 - 16:00, with occasional late shifts of 10:00 – 18:00 on a rotational basis. Weekend working is generally on a 1:4 ratio with days off during the week.
The team support patients registered with GP practices working in the Maidstone town and outlying villages within the ABC Primary Care Network.
Each team has two senior nurses to take responsibility for a defined geographical area, and provide clinical leadership to their Band 5, Band 4 and Band 3 staff. The team will work together to manage caseload patients delivering a high-quality service.
Staff will be required to drive their own car and are able to claim mileage costs and access the NHS car lease provision, following their probationary period.
Using the named nurse model, we want to allow nurses to spend more time with patients, promoting self- management where possible, and improving patient lives.
