Description:
EDUCATION, EXPERIENCE AND SPECIFIC REQUIREMENTS:
- Bachelor’s or Master’s degree in social work, counselling or other university degree relevant to the practice of counselling and psychotherapy from a recognized institution
- Member in good standing of a Professional College (OCSWSSW or CRPO ) or in process ofregistration
- A minimum of three years of relevant experience
- In-depth understanding of mental health and addiction issues
- Ability to refer children with mental health and addiction problems to appropriate services
- Knowledgeable on the mental health community resources available
- Commitment to working cooperatively with clients and members of the interdisciplinary team
- Openness to the customs and habits of other cultures
- Excellent communication and interpersonal skills
- Knowledge and ability to use basic computer software
- Excellent knowledge of oral and written French and English
- Be willing to work flexible hours to meet the needs of clients
- Have access to a motor vehicle and hold a valid Ontario driver’s license

Responsibilities:
- Process applications to the child and youth mental health program according to established procedures
- Assess strengths and needs of children, youth, and parents using questionnaires or standardized assessment tools such as the CHMYN (including monitoring and closure)
- Prepare service plans for children and youth served based on available reports, initial parent/youth meetings, and CHMYN assessments and other assessment tools as required
- Provide counselling, education and support services to maintain and promote emotional, social, behavioural and relationship functioning and wellbeing in the context of daily life
- Conduct case presentations to the mental health team with the clinical manager according to established procedures
- Consult the clinical manager during the probation period and according to the requirements of the professional college
- Ensure that all client interventions are evaluated following the closure of a file and that the results are noted
- Ensure that a service plan is developed and reviewed regularly
- Ensure that summary reports are completed prior to the transfer of a file and the closure of the intervention
- Deliver psychoeducational programs in response to requests received
- Refer families/youth to specialized services as needed
- Supporting families, children and youth who require specialized services
- Ensure coordination and case management when other partners/agencies are involved with the client’s file in accordance with established procedures
- Ensure that the approaches and/or programs recommended in the service plan are evidence based
- Develop and facilitate educational and support groups for children, adolescents and their parents according to the needs of the clientele
- Organizing workshops and activities to raise awareness of mental health issues
- Serve on community tables or committees to communicate the needs of the Centre’s clients
- Share information about the activities of external committees with the mental health team,
- Liaise and make referrals to community agencies to improve access to French language services
- Prepares reports and maintains records
- Maintain program statistics for evaluation and research purposes
- Ensure health promotion in the francophone community by aiming for the overall development of community health that includes all aspects of an individual’s life.
