Description:

Patient Care Communicate effectively with patients, relatives and carers, respecting patient confidentiality with privacy and respect for diverse cultural backgrounds and requirements. Communicate complex, sensitive or contentious information to patients, relatives or carers, where persuasive, motivational, negotiating and empathetic skills are required. Work as part of the multidisciplinary team to deliver evidence based cost effective care. Work in partnership with senior members of the team to review the delegated caseload and ensure the care provided is of high quality.
Work with patients towards self-care and independence utilising health coaching techniques Work to support families and carers. Be aware of the effects of illness on the family. Work as part of the multidisciplinary team to deliver supportive care to patients approaching end of Life. Support the admission and discharge process by liaising with patients and families, completing timely home visits, liaising with families re access and equipment delivery.
Plan and organise a range of clinical activities to ensure best practice is delivered. Take, record and report blood pressure, respiration, pulse rate and oxygen saturation levels as necessary. Be able to recognise the deteriorating condition. Complete washing and dressing assessments, kitchen assessments.
Refer to social care services as required. Discharge patients from service. Arrange care packages in accordance with protocol. Prescribe exercise and mobility programmes within the scope of practice.
Assess level of support required with medication, Administer, Assist, Prompt, None. Participate in the promotion of education and training to patients. Initiate, delegate and progress or adapt treatment plans for non-complex patients. Assist the senior staff with admissions.
Assess, provide and fit small aids and equipment within the protocols. Order therapeutic and rehabilitative equipment. Complete access, home, discharge home visits safely. Take responsibility for the safe use of clinical equipment.
Team Working and Development Contribute to, support and promote ECCHs, strategic direction, values and culture in relation to proactive and reactive services. Participate in clinical supervision within the team as required. Participate and maintain a learning environment and maximise opportunities for education and development in the clinical area.to enhance individual development and performance in the delivery of high standards of care. Contribute to an environment conducive to effective working, respecting and supporting staff to deliver high quality clinical services.
Contribute to change and innovation within the team. Engage in personal development; maintain competence, knowledge and skills commensurate with role. Be wholly accountable for practice taking every reasonable opportunity to sustain and improve knowledge and competence. Contribute to the effective use of resources.
To support evidence to meet the Care Quality Commission (CQC) fundamental standards. Adhere to Organisational Policies and guidance. Clinical Governance Contribute to clinical governance reports covering all aspects of activity. Actively participates in audits, monitoring of care and reporting of activity as requested, and as a result of the team identifying areas of concern, or areas for development.
Ensure that evidence is available and updated to demonstrate the Care Quality Commission fundamental standards are being met. To be able to maintain legible, accurate and contemporaneous patient records in accordance with ECCH Policy. Professional/Personal Maintain own professional integrity, being respectful and honest at all times. Maintain own knowledge and skills in order to be an effective clinician.
Work positively with the Multidisciplinary Team ensuring that the team focus on the needs of the patient. Professional Development and Education Ensure that the clinical environment and team promote learning and development of each other and visiting students of all disciplines. Encourage and facilitate team members to develop professionally and meet the competencies set for their role. Participate in own appraisal.
Communication and Interpersonal Relationships Competent with Information Technology (IT), systm one or generic case management software and able to manage own emails and online training. Actively participate in projects across the Organisation. Other Responsibilities The post holder will be expected to participate in a rota system of shift working to cover a 6- day a week service.
