Description:
Trainee Advanced Clinical Practitioner (TACP) Base Location: King Street WIC & various Local Care Direct (LCD) sites Line Manager: Clinical Education Lead Hours of Work: Various shifts with a minimum of 30 hrs up to 37.5 hrs as per apprenticeship agreement DBS Required: Enhanced with Child & Adult checks Vaccination Requirement: Covid19 double vaccination recommended. This job has been rated as medium risk for requiring Hep B immunisation & vaccination is recommended, therefore post holders will be offered Hep B. Flu jab recommended & offered. Job Purpose: The TACP will be registered with a professional body relevant to the training post, with relevant post graduate qualifications & clinical experience (minimum of 3 years post registration).
They will act within their professional boundaries & will provide care for the patient from initial history taking, clinical assessment, diagnosis & treatment to discharge or onwards referral. This may take place over the telephone, in a clinic setting or the patients home. They will demonstrate safe, clinical decision making & provide expert care for patients, working within their scope of clinical competence. They will be required to work flexibly to meet service demand.
The TACP will undertake the ACP apprenticeship course in order to fulfil the training post requirements with Bradford University. The ACP apprenticeship course is typically a 3 year training programme & the post holder will be based as a paid LCD employee. The apprenticeship encompasses work based academic learning, contributing towards gaining a masters degree in advanced clinical practice at a Higher Education Institute (HEI), alongside successfully completing a portfolio & a local competency framework of evidence based practice & consolidation of knowledge. They will work as a trainee under the supervision of experienced Clinical Educators to provide complete care ensuring safe & effective management of patients.
They will develop & build their experience & skills to deliver specialist knowledge, underpinned by Health Education Englands 4 pillars of advanced clinical practice. The TACP will act as a role model to develop the knowledge to deliver advanced practice skills within LCD, promoting patient centred clinical effectiveness, developing the skills to work autonomously in delivering independent assessment of undifferentiated & undiagnosed presentations. This will include developing the skills to perform clinical examination, diagnosis, clinical decision making & planning of care & treatment, diagnosis, prescribing, health promotion & safe discharge of patients. At the end of the apprenticeship & on successful completion of the end point assessment you will be eligible to apply for ACP roles advertised across LCD.
KEY PRINCIPLES Undertake & complete the acquisition & development of new skills, knowledge & experience based on advanced practice competencies which are underpinned by Health Education England 4 pillars of advanced practice. The TACP will complete a supported package of generic and specialist advanced practice competencies to develop skills which may include: Clinical Deliver & participate in the clinical care of patients attending the clinical area. Throughout the programme develop the skills of an autonomous practitioner ensuring that evidence based practice is inherent in all aspects of care Provide a level of advanced & comprehensive health assessment based on a specialist body of knowledge under direct or indirect supervision. Clinically examine & assess patient needs holistically & plan clinical care accordingly.
Provide assessment, treatment, & diagnosis at the point of contact which may be in a variety of clinical & non clinical settings. Make professionally autonomous decisions for which the post holder is accountable. Only work within the clinical competencies & scope of practice of the individual maintaining a positive approach to patient care & experience whilst recognising limitations of the training role Work under supervision with confidence & competence within professional guidelines. Complete & maintain accurate, contemporaneous records which include the documentation of the presenting history, past medical history, medications, allergies, examination findings, differential diagnosis & management plan & appropriate safety netting.
Adhere to Medicines management policy & procedures. work within parameters of current nurse prescribing legislation if appropriate or approved Patient Group Directives to initiate pharmacological interventions. Act as a referral agent & care co-ordinator by establishing multi-agency collaboration across the primary / secondary interface to meet the needs of the patient. Work within the Code of Professional Conduct or guidelines of relevant professional body e.g.
NMC, GPhC or HCPC. Research & Quality Demonstrate evidence-based care, working within agreed guidelines & protocols. Take an active role in research studies within the policy framework of the organisation. Participate in audit, data collection & activity monitoring to improve performance & inform future service development.
Choose an area of special interest within advanced clinical practice with the intention to develop & take forward in your ACP role. Professional & Education Take responsibility for own development, learning & performance ensuring course requirements are fulfilled. Work within the professional Code of Conduct & other directives as defined by the HCPC, GPhC or NMC. Undertake appropriate personal & professional development to meet the requirements of HCPC, GPhC or NMC registration.
Develop own skills & competencies through education, training & development activities related to the current & future demands of the role opportunities & where possible achieve Independent Prescriber status. Observe & participate in all areas of clinical practice including telephone assessment, face to face & home visits. Working flexibly to meet demand on the day. Understand & observe professional & national guidelines on confidentiality.
Comply with the corporate governance structure in keeping with the principles & standards set by the organisation. Ensure clinical supervision needs are met. Participate in teaching & mentoring other health professionals & members of the multi disciplinary team. Management Actively promote integrated working in collaboration with other health care professionals both within & outside of LCD.
Contribute to the development, implementation, review & evaluation of clinical protocols. Ensure effective communication, participating in meetings & consulting with other professionals, services & agencies as required. Maintain personal & professional development through evidence of continuous professional development (CPD), including demonstrating a personal awareness of advisory papers & current national issues that may have a local impact for the service. Participate in development of the UCP role & other healthcare professionals though teaching, mentoring & assessment; this will include the provision of evidence based healthcare advice which contributes to the diagnosis, care, or treatment of a wide range of clinical conditions.
Participate in the mentoring of colleagues & participate in the delivery of training material. Manage the safeguarding of adults & children & refer appropriately. COMMUNICATIONS & RELATIONSHIPS Ensure agreed lines of communication & reporting are implemented within the team to support all employees understanding LCD business plans, objectives, priorities & changes. Promote effective cross functional working & that the operational team supports the achievement of objectives, standards & protocols of other LCD areas.
Attend internal & external meetings representing LCD & engaging in discussions appropriate to the role and level of the post. PATIENT CARE Ensure that the quality of patient care is firmly at the centre of the organisations objectives & vision. Ensure the management & monitoring of the highest standards or safety and quality in the delivery of healthcare and related services and manage risk within LCD. FINANCIAL RESOURCES Ensure financial policies & procedures are maintained & the budget is adhered to and that best value for money is achieved.
CONFIDENTIALITY Operate & ensure the team operates within information governance protocols & that business confidentiality is maintained at all times. TRAINING Complete the defined company induction programme & core training for this role & mandatory refresher training thereafter & ensure that accurate records of training are maintained. INFECTION PREVENTION AND CONTROL (IP&C) Healthcare workers have a duty of care to patients & are expected to comply fully with the companies IP&C policies & attend or complete IP&C training, for personal & patient safety & for prevention of healthcare associated infection.
