Trainee Advanced Clinical Practitioner Stroke Medicine

Nottingham University Hospitals (NUH) is nationally recognised as having an established Advanced Clinical Practitioner (ACP) service, with ACPs embedded across 18 specialties. We are now seeking committed, enthusiastic and compassionate trainee ACP to undertake the three year apprenticeship at the University of Nottingham and join our established Stroke ACP team.
The Stroke department is led by a group of senior Stroke Consultants, and delivers secondary and tertiary care. Our team of experienced Stroke ACPs provide cover and leadership to the TIA clinic and Stroke Same Day Emergency Care Unit. The SDEC Unit was set up and is primarily staffed by ACPs providing rapid assessment of patients alongside an experienced nursing team. We also offer an in-reach service to ED to provide autonomous early intervention to patients who present at the QMC campus. Our goal is to provide the best possible care in this busy and thriving department. The ACP can expect to encounter conditions ischaemic stroke, haemorrhagic stroke, transient ischaemic attacks and stroke mimics.
We also provide support to the hyper-acute stroke unit, acute stroke unit and lead the stroke rehabilitation ward.
The post holders will be based at Nottingham University Hospital NHS Trust, at both QMC and City Hospital sites.
The successful candidate will undertake training to develop specialist knowledge and skills to provide healthcare autonomously to patients in their area. The ACP is accountable for independent clinical assessment, diagnosis and treatment of patients with undiagnosed and undifferentiated conditions.
The post holder will be a 1st level registered nurse or registered healthcare professional and work towards a MSc in Advanced Clinical Practice at Nottingham University. They will undertake an independent Non-Medical Prescribing course. They will gain knowledge and skills to practice at an advanced level demonstrating in depth knowledge and competence in all aspects of medicine, encompassing advanced clinical assessment, critical thinking and clinical management skills that are evidence based and deliver high quality patient centred care.
Once trained, the post holder will have a Trust-wide responsibility to promote clinical excellence in the care of patients presenting to their area by providing clinical advice and support to nursing staff and other health care professionals. They will provide expert professional and independent clinical care, which enables the coordination of a multi professional seamless service for patients. As a senior member of the team, the ACP will also play a pivotal role in the operational development of their specialty.

This post holder will undertake training in order to deliver on all four Pillars of the ACP role; Clinical practice and expertise, Leadership and management, education and research.
Uses highly developed clinical knowledge to independently assess, diagnose, plan, implement and evaluate treatments and interventions for patients presenting to the specialty with complex undifferentiated or undefined
presentations.
Undertakes physical examination as indicated by the patient’s condition; autonomously analysing complex clinical signs and investigation results to consider differential diagnoses and diagnose conditions, initiating treatment as
required.
Analyses multiple sources of data including patient history, physical examination and investigation findings when
making diagnoses, clinical judgements and evaluating care provided; presenting this information, to speciality/senior medical staff for advice when scope of practice is exceeded.
Works in partnership with the existing senior medical, nursing and AHP teams in driving departmental development and change.
Takes responsibility for their own learning and performance including the participation in clinical supervision and
maintaining an awareness of relevant research evidence.
