The One Coventry Integrated Team is pleased to offer an exciting opportunity for an enthusiastic Allied Health Professional or Registered General Nurse to support the Coventry prehospital and consultant led pathways as part of wider admission avoidance strategies.
This job profile is for a trainee Advanced Clinical Practitioner (ACP) position, who will work under clinical supervision to meet the capabilities/competencies required for the job, outlined below and progressing to a Band 8A on completion of the Advanced Practice Level 7 course. The trainee is required to work a minimum of 30 hours (0.8 WTE) as per Health Education / NHS England guidance.
Specialty specific competencies/capabilities must also be achieved alongside successful completion of MSc Advance Practice Programme identified in Person Specification deemed necessary for autonomous working within a five-year time frame. Competence in Non-Medical Prescribing will be required, if supported by the practitioner’s professional regulator.
The successful applicant will be an experienced healthcare practitioner who, will be eligible and willing to work towards qualifications to support role development and act within their professional boundaries and within an agreed scope of practice, to provide care (with robust clinical oversight and support from established Clinical Practitioners, Advanced Clinical Practitioners, and a Nurse Consultant within the Community Urgent Response Team).
As you progress through the health assessment module and achieve key competencies, the role will include initial/comprehensive history taking, clinical assessment, diagnosis, treatment (including use of expert knowledge and clinical judgement to decide whether to refer patients for investigations) and evaluation of care for those patient’s presenting with acute/undifferentiated conditions, supporting the

Co-ordinate the community care of patients throughout their consultant led and/or prehospital pathway with the multidisciplinary team and clinical services to ensure the effective management of this group of patients.
Accountable for own actions in accordance with the professional Code of Conduct and regulatory guidance. Will contribute to corporate objectives, acting within local, Trust and statutory guidelines and policies at all times.
Lead and deliver care across a variety of clinical settings such as community, outpatient/ virtual clinic, virtual ward rounds, front door etc
Deliver clinical care according to national and local guidelines ensuring delivery of evidence-based care and working towards the Trust vision of providing a World Class Service.
Assess and prioritise patients within the service to maintain quality care.
Undertake an initial assessment to determine differential diagnosis, utilising advanced critical thinking and decision-making skills, deciding when necessary to refer to senior clinical colleagues.
Act autonomously to provide expert clinical care which contributes to the patients’ treatment plan to ensure that optimum physical, psychological, and social care needs are met.
Request and interpret relevant investigations to facilitate the development of the patient’s management plan.
Proactively refer to other clinicians (own or other services) depending upon patients’ needs to deliver care beyond your limitations and scope of practice.
Proactively initiate treatment plans for patients requiring intervention and treatment ensuring ethically based considerations.
Proactively identify, diagnose, and manage treatment plans for patients at risk of developing long term conditions.
Undertake relevant clinical procedures to facilitate patient care within the service, supported by the appropriate training and supervision.
Assess, diagnose, prescribe (as per scope of practice and professional regulatory requirements), and review medications for therapeutic effectiveness in the context of acute/emergency illness/ chronic illness, considering any current treatment given including pre-existing medical conditions.
Providing information and advice to patients on prescribed medication regimens, side-effects, and interactions ensuring compliance with evidence- based practice and national and practice protocols, in accordance with their scope of practice and professional regulatory requirements
Use the SBAR tool to communicate clinical concerns to the senior medical staff if a patient is found to be critically unwell or deteriorating clinically.
Communicate complex information effectively to patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, including religious, cultural, and sexual orientation, and preferred ways of communicating are respected.
Maintain effective communication with the multidisciplinary team, stakeholders and other services including providing constructive feedback as appropriate.
Demonstrate advanced communication skills in breaking bad news and ability to support patients, family, and staff.
Anticipate barriers to communication and take action to improve communication.
Act as an advocate for patients and colleagues.
Access sources of support (e.g., PALS), guidance and information in an acceptable format to all patients, recognising any difficulties and referring appropriately.
Act as a credible, professional, and effective leader.
Deliver care according to local, national guidelines and quality standards to ensure delivery of evidence-based, safe, quality care.
Demonstrate continual evaluation of practice within the defined specialist area, making agreed changes where appropriate.
Critically evaluate and review innovations and developments that are relevant to the area of work.
Provide leadership to appropriate staff in the service to deliver high quality and effective care, ensuring the needs of patients are a priority.
Contribute to the appraisal of the team, providing feedback as appropriate.
Monitor the performance of team members, identifying and implement actions to achieve/maintain standards of care in line with organisational policy.
