Description:

This an opportunity for an experienced healthcare professional with extensive experience of the transfer of care process to lead the transfer of care team at PAH.
The post holder will be responsible for the management and smooth running of the Transfer of care team reporting to the Transfer of care hub lead working within the care co ordination centre.
The successful candidate will be responsible for ensuring good working relationship and lines of communication are maintained across the wider system to allow good patient flow throughout the hospital and into the community. ensuring accurate data is shared with the wider system.
The team work across site visiting and working and reporting to our internal stakeholders and external system partners within the west Essex and east and north Herts areas
The post holder will be responsible for the management and smooth running of the Transfer of care team reporting to the Transfer of care hub lead working within the care co ordination centre.

The post holder will assist and provide a pro-active and efficient transfer facilitation service and leadership of the Transfer of care Team within PAH. They will provide education and advice to maintain a high standard of Discharge Planning processes. They will work collaboratively within the Transfer of care Team to ensure and promote the timely access to health and social care resources to enable the safe and effective discharge of patients from the Trust.
The post holder will be the clinical lead for the Continuing Heath Care Assessment and discharge to assess process within the trust.
They will work in partnership with other inter-dependent teams and services across the health and social care economy that have influence and involvement in the transfer of patients from the trust.
The post holder will be responsible for proactively managing the local resolution of incidents and concerns, where necessary formally investigating incidents, complaints, errors or omissions in respect of patients, visitors, and staff with the Service leads from the Integrated Hospital Discharge Team and Head of Transfer of Care.
The post holder will provide proactive empowering leadership to the Transfer of care team; holding direct line manager responsibilities for the Discharge Facilitators, assistant service manager and transfer of care co coordinator
