Description:
JOB SUMMARY To lead the transformation of community services focusing on the east Suffolk service model. The post holder will be responsible for the delivery of the project in line with the Hospices strategy and business plan. The post holder will engage with colleagues across all areas of the Hospice, bringing together expertise, skills and ideas to support transformation. KEY ACCOUNTABILITIES To lead and monitor the community transformation project working with the wider project team and key stakeholders.
Ensure there is a comprehensive project plan and associated project management documentation to ensure the project is delivered in line with the ambitions set out in the project initiation document. Ensure the delivery of project outcomes which are aligned with the Hospice strategy and business plan as well as local and national relevant strategies. Identify project risks and interdependencies, formulating plans to manage these and determining future resource requirements. Use data to support a population health approach and future workforce development.
Work with our Alliance partners to ensure the service model is fully integrated with other services and initiatives. Promote and support a culture of quality improvement, using recognised quality improvement methodology. Develop and implement effective risk management strategies to mitigate risks and protect the safety of patients, staff, and visitors, including infection prevention and control. Champion a culture of inclusion by developing and implementing strategies that enable all individuals, regardless of background, to have equal access to, and benefit from our services.
Collaborate with service leads for Great Yarmouth and Waveney to ensure a coordinated approach to service delivery across all of our services. Develop mechanisms for reporting the progress of the project and work with the business intelligence team to ensure key metrics are developed and reported on to evidence the activity and effectiveness of the service model. Responsible for planning and advising on the co-production and implementation of a new service model and pathways to achieve improvements in outcomes and patient experience. Develop and present associated business cases and policies to support a revised service model.
Provide line management to the compassionate community lead to ensure the principles of the compassionate community philosophy are embedded in the revised service model. GENERAL Where you are a member of a professional body you are required to conform to the professional standards set by that body. You are required to ensure your registration is current and practice continuous professional development; Because of the special nature of the hospice and its work the post holder may on occasion be asked to undertake other duties to help maintain our high standard of care, to uphold the hospice values at all times
