Description:
HOW YOU’LL MAKE A DIFFERENCE
As a Transition Assistant, you will be working as part of our Preparing for Adulthood Team, based in Kingswood. This team sits within the 0-25 Disability Service at South Gloucestershire Council.
You will work with children and young people aged 13-25 who have a learning or physical disability and have an Educational Health Care Plan (EHCP) or an allocated worker form social care.
WHAT WE NEED FROM YOU
- With previous knowledge and experience working with professional care groups and external contacts, you will be able to manage challenging behaviours and show sensitivity and empathy in your work.
- Educated to a vocational qualification e.g. QCF/NVQ 2 or equivalent is desirable, you will be expected to acquire this if not already held.
- You will demonstrate knowledge of relevant legislation, regulations and codes of practice along with experience of specialist equipment and an awareness of “Manual Handling” techniques.
- There may be a requirement to travel widely within the authority, so you must either have a current driving licence and provide a car or have access to appropriate means of travel.
Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained for this post.
Responsibilities:
- You will be responsible for providing individual support to a child or young person contributing to them being able to access services available in their local community.
- You will help a child/young person with travel requirements or be involved in a school travel training project, ensuring the service of vehicles meet the required workplace protocols and vehicle checks.
- With the support and direction of the Transitions Facilitator, you will be responsible for completing and maintaining records on Mosaic – our case management system.
- Occasionally you may need to support or assist in the administration of medication for a child/ young person or their personal care needs. You will undertake support work in accordance with Social Care National Occupational standards and NICE standards.
