PERMANENT PART TIME – 40 HOURS PER FORTNIGHT

This is an exciting opportunity for a suitably qualified Registered Nurse or Allied Health Professional to join the Community Services team as Transition Care Program (TCP) Coordinator. In this role you will manage the coordination and development of Maryborough District Health Service’s Transition Care Program (TCP). Key responsibilities include managing day-to-day operations of the TCP, ensuring the program meets all regulatory and funding requirements, developing and implementing individualized care plans for clients transitioning from hospital to home or other care settings, liaising with healthcare professionals, clients, and their families to ensure seamless transitions, and monitoring and evaluating the effectiveness of the program to ensure continuous improvement. The ideal candidate will have a strong background in healthcare management, excellent communication and organizational skills, and a passion for improving patient outcomes during transitional care periods.
