Stonehenge delivers long-term, intensive residential treatment services to those whose addiction/concurrent presentations are complex. Many of our clients enter treatment directly from the Federal and Provincial correctional systems.

QUALIFICATIONS:

  • Completion of a University Degree relevant to mental health, addiction, or Social Services
  • Minimum one year of progressive experience in addictions or mental health; experience in concurrent disorders or trauma informed practice an asset
  • Certified Addictions Counsellor or equivalent an asset
  • Previous experience in residential treatment an asset
  • Previous experience with justice-involved individuals preferred
  • Previous experience in correctional field an asset
  • Food Handlers Certification an asset
  • Lived experience related to addictions and/or mental health an asset
  • Valid G Driver’s License for a minimum of 3 years (G2 or greater)
  • Satisfactory Motor Vehicle Record
  • Reliable vehicle/own transportation – not on a bus route
  • CSC Clearance conducted by PWGS Canada
  • Mandatory on call duty, on a rotational basis
  • Standard First Aid/CPR Level C
  • Must be able to attend work (travel to Residential Centres) within an hour of notification, when on call

Responsibilities:

ABOUT THE ROLE:

In a Residential treatment environment, staff work with individuals who have acute and/or chronic addiction issues and concurrent issues. As part of our program, staff become part of the overall “Community”, and engage in community living activities with our residents including mealtimes and recreation activities.
The Transition Coordinator’s primary responsibilities are to provide group support to Residents that will soon be leaving the Residential program, facilitate intakes into the program, and to support residents through all transition phases of the program. Additionally, the Transition Coordinator will support with ensuring the Therapeutic Community model is upheld at all times by providing in the moment feedback to staff and residents.
Stonehenge is one of only two funded Therapeutic Communities in Ontario. This is an excellent opportunity for those interested in working with a specialized approach to addiction treatment and recovery from substance use.

ROLE OVERVIEW:

Coordination of Resident Intake and Discharge:

  • Coordinate and support intake and orientation of new residents
  • Conduct and support planned and unplanned resident discharges
  • Complete relevant documentation
  • Plan and conduct resident completions

Coordinate and Support Residential “Aftercare” Program Support:

  • Facilitate the weekly support group at the Administration building
  • Provide individual counseling, as needed
  • Utilize evidence-informed practices including (but not limited to) Motivational Interviewing, Stage-Matched Intervention, Trauma-Informed approaches, CBT, and DBT
  • Guide participants to maintain a healthy and balanced lifestyle
  • Advise team of re-enhancement and re-grounding requests and recommend as necessary; Complete intake for re-grounding residents
  • Attend all commencement ceremonies with Aftercare participants and select speakers
  • Maintain appropriate documentation
  • Monitor and screen clients for abstinence during group attendance; Manage incidents of intoxication
  • Maintain professional contact with clients via STC email for announcements, program changes, or support
  • Liaise with Probation and/or Parole Officers when necessary and report any required attendees and/or disclose any drug use with the direction of the Clinical Manager – Residential Services
  • Provide and receive delegation in partnership with other Residential Staff
  • Make referrals to internal and/or external programs or services, as appropriate

Support Residential Program “Re-Entry” Work:

  • Develop individual transition plans with residents and complete required assessments, reports, and documentation
  • Plan, organize, and deliver group therapy focused on reintegration of residents into the outside community
  • Develop comprehensive relapse prevention plans with the residents
  • Develop housing, employment, and leisure plans with the residents, connecting them to community agencies and supports
  • Support and monitor resident leisure/business passes into the community, liaising with family members, correctional, and community agencies

Other Residential Program Responsibilities:

  • Respond to crisis and urgent situations
  • Conduct educational presentations with residents, as required
  • Monitor resident progress and coordinate resident transitions
  • Transport residents to appointments, hospitals, etc. in agency vehicle, as required
  • Participate in program development, evaluation, and complete statistics
  • On a rotational basis, provide on-call services during evenings and weekends
  • Attend both clinical and administrative meetings
  • Participate in supervision
  • Provide peer mentorship and coaching to the team
  • Conduct/inform staff briefing in between shifts
  • Provide light house keeping
  • Facilitates medication administration
  • Complete resident documentation
  • Coordination of outings
  • Make referrals to internal and/or external programs or services, as appropriate
  • In accordance with a Therapeutic Community Treatment Model, make decisions with a team-based approach
  • Other duties as assigned
