Description:

THIS POST IS FIXED TERM FOR 17 MONTHS DUE TO FUNDING.

An exciting opportunity has arisen for a Triage Nurse to provide support to the newly established Single Point of Contact team for the 3Ps: Promote, Prevent and Prepare Programme.
The programme consists of six steps across promotion of wellbeing, prevention of deteriorating conditions and health and preparation of patients for surgery to reduce DNAs, adverse outcomes following surgery and reduce further medical intervention.
The post holder will contribute towards providing advice and guidance for moderate risk patients on waiting lists and will act as a resource that members of the team can escalate patient queries to, where appropriate.
They will also support both non-clinical call handers and senior clinical members of staff in the team and signpost and escalate patients to prevent deterioration of conditions whilst on waiting lists.

The post holder will provide specialist nursing care, advice, guidance and leadership, and take responsibility for a specialist nursing service using their advanced clinical knowledge and experience with a high level of skill and theoretical knowledge to;

  • Assess, plan, implement and evaluate evidenced-based nursing care.
  • Provide expert advice and clinical practice.
  • Work autonomously and manage own workload as a lead specialist in their area Provide specialist education and training to Staff
  • Lead and manage a team of specialist nurses
  • Work collaboratively and co-operatively with clinical colleagues to develop services and the quality of care delivered.
  • Lead the development and planning of clinical services within the specialist field.
  • Demonstrate the Health Board’s values and ensure that others demonstrate the same values.
  • Undertake clinical Activity e.g. Clinics, ward-based work, telephone consultations etc.
  • Undertake professional Activity e.g. Audit, CPD, teaching, research and administration
