UNUSUAL INCIDENT REPORT (UIR) DOCUMENT COORDINATOR

J ob Location: Hamilton, NJ (Mercer County)
Hours: M onday-Friday
Position Type: Full-Time, Flexible Schedule, Exempt

REQUIREMENTS:

  • High School Diploma or equivalent with at least three (3) years of relevant experience, i.e. either investigative or I/DD experience.
  • Valid unrestricted driver’s license with good driving record.
  • Ability to lift at least 15 pounds.
  • Excellent oral and written skills.
  • Computer Proficiency
  • Independent judgment is required to plan, prioritize, and organize diversified workload.

Responsibilities Include:

  • Under the guidance of the Senior Director of Incident Investigations or designee, receive UIRs from DDD and DCF residential and vocational programs and report them as outlined in applicable state laws and/or regulations.
  • Log and report daily UIR’s in accordance with NJ DDD and DCF standards.
  • Ensure the accuracy of the information being reported; and if applicable, resolve discrepancies in the information received and obtain additional information for incomplete reports.
  • Send out all applicable correspondence related to the daily UIR’s.
  • Enter data into the state Office of Program Integrity and Accountability (OPIA) and Allies Tracker software, ensuring the accuracy of the data being entered.
  • At times follow-up on all pending UIRs and close them in a timely fashion, specifically in relation to the Critical Incident Management Unit (CIMU) under the auspices of the OPIA.
  • May be required to participate in investigations and write closing reports while adhering to both standardized Investigation Practices as well as the Allies, Inc. UIR Investigation Policy & Procedures.
  • Will assist the Senior Director of Incident Investigations, or designee, with compiling, analyzing, and reporting on trends that may pose future risks hazards to the company and its employees.
  • Provides all documentation to relevant departments within Allies such as Human Resources, Payroll, Maintenance, Clinical team, when applicable.
  • Provides follow-up and consults, as needed, to Human Resources, Residential Management, and/or Administration of Allies, Inc.
  • Provides follow-up for any outside Agency as designated by the Senior Director of Incident Investigations or designee.
  • Exercises strict confidentiality in all reports and investigations.
  • Other duties and Special Projects as assigned.

PURPOSE:

The UIR Document Coordinator will assist in reporting, following up as needed, and tracking State of NJ Office of Licensing mandated Unusual Incident Reports from all residential and vocational programs in Allies Inc. Will also assist the Senior Director of Incident Investigations in further utilizing this data to determine areas to improve training for staff in residential and vocational programs. This employee may also be required to assist the Compliance Department with additional matters and to consult with Allies Affiliates as needed.
