Job Description
The Saskatchewan Health Authority (SHA) - Saskatoon, Primary Health seek a family physician with an interest in geriatrics to work with the Seniors First Program.
Seniors First is an enhancement of existing home and geriatric services within the SHA-Saskatoon. It is not intended to replace other community based services. The focus of Seniors First’s interdisciplinary team is to support frail older adults to remain at home and age in place, to and facilitate early discharge from hospital; prevent hospital admissions; reduce unnecessary visits to the Emergency Department.
The successful applicant will work with older adults with a focus on the frail elderly over the age of 85, who: have complex chronic health needs; are at risk of hospitalization or readmission; consent to having services at home and live in Saskatoon.
Preference will be given to candidates with a background in geriatric medicine. The physician will be working in partnership with community physicians and programs, and in an interdisciplinary team-based care environment. Team members include home care, nurse practitioners, geriatric psychiatry, paramedics, occupational therapists, physical therapists, pharmacists, CPAS, social work, and many others.

Job Qualifications
Successful applicants will be eligible for Canadian certification in family medicine, licensure with the College of Physicians and Surgeons of Saskatchewan (CPSS) and membership on the SHA-Saskatoon Practitioner Staff.
