We’re looking for Band 6 Urgent Care Practitioners to join our Crisis Resolution and Home Treatment Team (CRHT)
The Urgent Care Mental Health Practitioner is required to prioritise and respond to the needs of patients referred to the Urgent Care Service according to presenting level of urgency so as to manage safety of patients and agreed response times.
To carry out psychosocial assessment, including risk assessment and to formulate a clinically appropriate onward plan of care, providing specialist mental health advice, support and interventions to staff, carers and patients and other agencies. They will provide clinical supervision to staff and students.
This post offers the Trust’s £4,000 Incentive for band 5-7 Nurses & Practitioners, as well as a relocation package up to a maximum of £8,000 (subject to terms and conditions).
To provide intensive and responsive crisis resolution and home treatment to adults aged 18 and over with a functional mental health problem, ensuing individuals are treated in the least restrictive fashion which is consistent with their clinical and safety needs.
- Regularly undertake the role of leading and co-ordinating the teams daily activity to ensure continuous and safe support of all patients referred to CRHT case load.
- Taking referrals directly from referrers, screening referrals, accepting and declining referrals (based on criteria) and arranging appropriate initial response to referrals.
- To work closely and effectively with inpatient units to facilitate effective and timely discharge for patients who without CRHT would remain inpatient
- To assess and manage ongoing risks as identified during the assessment ensuring that this is done in collaboration with the patients and cares.
- To ensure clinical practice is evidence based and consisted with relevant NICE guidelines and trust policies.
- To contribute to and support managers, colleagues and wider organisations in implementing service developments towards successful change.
- To apply highly specialist skills and knowledge in order to establish professional competence and fitness to practice.

REQUIREMENTS
In order to be a successful Urgent Care Practitioner in our team, you must:
Hold a current and relevant registration I.E Be a Registered Nurse, OT or Social Worker with current registration (Adult/Mental Health/LD or equivalent) appropriate to the job role
Significant post qualifying experience in the relevant care group
Experience of working alongside people with complex health problems in the designated care group
Experience of conducting clinical assessments including risk assessments
IT WOULD BE ADVANTAGEOUS IS YOU HAVE:
Previous experience working in acute services, home treatment teams or Liaison Psychiatry.
