Description:
- Under the direction of the Urgent Community Response Nurse is charge you must assess, plan, implement and evaluate patient care demonstrating evidence based / best practice, considering own clinical confidence and competence. You must implement enhanced programs of care
- To be part of the Urgent Community Response Team to provide high standards of clinical intervention and to provide nursing care for service users accessing the range of services this includes acting as nurse co-ordinator as and when required.
- To implement enhanced clinical care programs that are of the highest quality & standards and in line with the Walsall Healthcare NHS Trust’s policies and procedures
- To work within a multi-agency / multi-disciplinary team adopting a ‘no blame’ culture
- To adhere to Walsall Healthcare NHS Trust policies, procedures and protocols to promote the quality of patient experience, ensuring the highest standard.
