Description:
An exciting opportunity has arisen for a Community Physiotherapist to join a new service in the Virtual Ward team for a 9 month secondment.
Virtual wards are a national initiative and are defined as “A safe and efficient alternative to NHS bedded care which is enabled by technology” by NHS England and NHS Improvement. Virtual Wards provide healthcare systems with a significant opportunity to delivery care through the collaboration of community services and acute hospitals providing an alternative to an acute admission and/or early discharge. Nottinghamshire Integrated care system, aim to deliver this through a Nottinghamshire wide provider collaborative, incorporating Sherwood Forest Hospitals, Nottingham University Hospitals, Doncaster & Bassetlaw Hospital, Nottinghamshire Healthcare and City Care.
The Nottingham and Nottinghamshire virtual ward is a new service which aims to provide an innovative, responsive, safe, effective, and caring service covering a variety of specialities including frailty, respiratory, palliative, stroke, acute medicine, orthopaedics, oncology and admissions avoidance. Patients receive the care they need via remote monitoring and face to face treatment in their own home, safely and conveniently rather than in hospital.
The Virtual ward position will be based in either the mid or North Nottinghamshire geographical patch.
A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
Virtual ward allow patients to receive the care they need at home (including care homes) safely and conveniently rather than in hospital for a short duration (up to 14 days).
All virtual ward staff are required to deliver person centred care, timely assessments and comprehensive oversight with the aim of preventing unnecessary hospital admissions. You will work closely in an MDT to support individuals on the virtual ward.
The virtual ward MDT consist of: Matrons, Nurses, Physiotherapists, occupational therapist, nurse associates and assistant practitioners.

We are looking to recruit a dynamic, flexible, and compassionate Senior Community physiotherapist to join this new and exciting service within the localities of Mid or North Nottinghamshire.
You will work within the virtual ward team and in collaboration with system partners such as City Care, as part of a supportive multi-disciplinary clinical team with a dedicated focus of delivering excellent patient centred care and innovation.
You will be providing care to patients in a community setting, who require high quality holistic evidence-based care that is responsive, effective, and efficient in line with the virtual ward model. This will be via remote monitoring or face to face assessments.

- You possess community experience or acute care experience with a passion for learning, demonstrate excellent communication and interpersonal skills with the ability to be adaptable, reliable, resourceful, and approachable, evidencing high levels of personal and clinical credibility, being able to instantly establish effective working relationships in a range of work settings.
- You have a thirst to use digital and remote technologies within your assessments.
- You demonstrate high levels of care and compassion to promote independence, prevent hospital admission or admission to care, support early discharge from hospital and to help people to receive the care they require within the places they call home.
- You enjoy working collaboratively across organisation boundaries, within a multidisciplinary framework, analyse and develop treatment plans whilst providing compassionate, realistic, and timely support to a diverse patient group and their families
