The Integrated Virtual Wards Team are looking for two enthusiastic and proactive nurses to support the growth and success of our Virtual Wards Programme. These roles will be crucial to the safe admission of patient onto our virtual wards and their monitoring during their stay.

Virtual wards are a safe and efficient alternative to NHS inpatient care. This helps prevent avoidable admissions and supports early discharge out of hospital, improving patient outcomes and experience, whilst also reducing pressure on acute care.
Carry out planned care for a group of patients without direct supervision, utilising escalation pathways to ensure care is coordinated into organisations in North West London.
Ensure the assessment, planning, implementation and evaluation of evidence based, individualised patient care, including supporting case finding, reviewing referrals from teams and onboarding to Virtual Wards pathways.
