In your new role as our Wellbeing and Transitions Manager, you will engage with current and prospective residents and families; advising on suitable care and service options and provide support through the resident’s journey with Australian Unity.

Responsibilities will include:

• Supporting planned and unplanned customer transitions and advising residents and families of the most suitable care/service options to meet their needs
• Providing a meaningful and seamless experience for the resident; focusing on their goals and facilitating a warm transition to care
• Maintaining and maximizing resident occupancy levels; monitoring referrals/sales inquiries and ensuring a high-level customer experience
• Developing strong relationships with the co-located Retirement Community and wider teams
• Building a strong referral network of service providers, health professionals, community groups and other community partners to support referral channels
• Supporting National Wellbeing and Transitions Manager with projects
• Remote support of leave coverage to the wider Wellbeing and Transitions team
