JOB SUMMARY

The Women’s Program Coordinator is responsible for the development of programs and implementation of programs, events, and services related to AID Atlanta’s Women’s Program. Additionally, the Women’s Program Coordinator provides interventions that help women to overcome barriers to care through culturally sensitive care coordination and support. The primary purpose of this position is to provide outreach, referrals, and support to women impacted by and at risk of HIV/AIDS with the goal of improving quality of life and health outcomes.

EDUCATION & WORK EXPERIENCE

  • Bachelor’s degree preferred, High school diploma or equivalent required
  • Experience in the field of HIV is preferred, but not required.
  • Proficient in using Microsoft Windows and Microsoft Office Applications, including Excel and Word; Proficiency with Internet and Social Media platforms
  • Ability to use Electronic Medical Records for documentation of services provided
  • Bilingual (English/Spanish) strongly preferred but not required
  • Must pass criminal background check

Responsibilities:

  • Coordinates outreach activities and special community events related to the Women’s Program under the direction of the Director of Client Services and/or Support Services Manager
  • Identifies community and subject experts who will participate in active engagement and

conversation during scheduled forums, events, and groups

  • Helps to identify specific settings or venues in which members of the target population can access programs and services
  • Educates members of the target population about AID Atlanta’s programs and various community services
  • Coordinates with community partners to address areas such as members’ personal and professional development, interpersonal relationships, and support networks.
  • Identifies and maintain active rosters of all active participants and up-to date resources
  • Perform timely and accurate input of all required information in EMR (data entry and supporting documentation) to ensure compliance with regulatory and contractual requirements.
  • Document all work done with and on behalf of members into agency and/or funder client database system in accordance with documentation standard (eCW, e2Fulton, etc)
  • Effectively coordinates with internal and external programs to ensure appropriate linkage to

services (i.e., Medical Care, PrEP, Testing, Intervention Workshops, etc.).

  • Compiles and creates educational and resource materials in user friendly formats

for distribution to program members

  • Always maintains member confidentiality in accordance with HIPAA guidelines, federal, state, and local laws, and AID Atlanta/AHF policy to ensure a high level of professionalism is maintained
  • May perform administrative support tasks, such as copying, transcribing handwritten

information, data entry, photocopying, scanning, etc.

  • Link members to various support services that aid in reducing barriers to care (i.e., health insurance enrollment, case management, financial assistance, transportation, and other social services)
  • Complete monthly reporting to ensure accurate programmatic reporting
  • Participate in and volunteer for intra and interdepartmental activities/events across the agency (i.e., Women’s Program events, AIDS WALK fundraising, AIDS 101 participation, HIV testing and counseling, etc.).
  • Communicate regularly with staff about changes, updates, and improvements in the Women’s Program in reference to service delivery issues that impact members and/or staff.
  • Attend agency, local, state, and national meetings, conferences, or workshops as needed or required.
  • Adhere to policies and procedures for AID Atlanta/AHF and other off-site programs to ensure quality standards are met.
  • Address members concerns utilizing the Customer Service Standards.
  • Perform routine self-audits and maintain charts to ensure members information and eligibility documents are current.
  • Meet with supervisor to review and discuss chart audits/observation results, and any related concerns regarding program goals and objectives
  • Participate in various trainings; internal, external, and online to maintain a knowledge base of program functions and to ensure best practices.
  • Coordinates education programs for women impacted by and at risk of HIV/AIDS with the goal of improving quality of life and health outcomes.
