Coordinate and collaborate with the interdisciplinary team to expedite medically appropriate, effective patient care. Conduct initial and concurrent reviews utilizing nursing, biological, pathophysiology, psychiatric and continuum of care knowledge and expertise and clinical utilization tools to apply medical appropriateness criteria to resource utilization and transition/discharge planning, advise the interdisciplinary team on appropriate level of care and discharge planning needs, intervene to address barriers, and provide leadership as needed to achieve optimal outcomes, including securing funding and approvals for services as required. About the Services at Joseph & Rosalie Segal & Family Health Centre - Mental Health & Substance Use Inpatient Units The Segal Psychiatric Inpatient Units are located on Levels 5 to 8 in the Joseph & Rosalie Segal & Family Health Centre at Vancouver General Hospital. Psychiatric inpatient services provide assessment and treatment to adults who require acute 24-hour management in a hospital setting. There are 75 beds focused on clients who require longer-term, intensive treatment to return to their community. The length of stay is approximately five to 12 days. The Segal Psychiatric Inpatient Units include: Brief Intervention Unit – Segal 5th floor Mood Disorders – Segal 6th floor Inpatient Psychiatry – Segal 7th floor Inpatient Psychiatry & Concurrent Disorders – Segal 8th floor About the Psychiatric Assessment Unit (PAU) at VGH The Psychiatric Assessment Unit at Vancouver General Hospital is a 14-bed short stay unit focused on clients who require short term, intensive treatment to return to their community, with length of stay being approximately three days. At PAU, Nurses support patients with their physical and mental health needs as well as day-to-day activities. The PAU team endeavors to provide a safe place that promotes acute psychiatric stabilization for patients in a comforting, caring and holistic environment. Qualifications Education & Experience Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three (3) years recent, related experience including (1) years’ experience in case management, discharge planning and/or utilization review, or an equivalent combination of education, training and experience. Knowledge & Abilities Knowledge of Nursing theory and practice within a patient/family centred model of care. Knowledge of the principles of recovery in mental illness. Knowledge of the BCCNM standards of practice for registered nurses and registered psychiatric nurses. Knowledge of evidence based nursing practice related to various patient population groups and demonstrated ability to apply knowledge to a case management process. Knowledge of clinical pathways, expected length of stays, resource utilization, and patient assessment. Knowledge of external agencies and community resources.
