The Team: The Discharge Planning Team are committed to delivering the best outcomes to patients across the North-West region. Demonstrating experience, understanding and expertise in the clinical management of patients with complex discharge requirements. The team works in partnership with relevant stakeholders such as Nurse Unit Managers, Nurse Manager Patient Flow, Allied Health and Community Liaison Nurse, to co-ordinate the discharge processes for patients within the hospital to ensure timely access to services and or care. The Role: As a member of the multidisciplinary team within Nursing Services, the Clinical Nurse Consultant - Discharge Planning provides clinical leadership and direction in the planning, implementing and evaluation of change management processes to ensure efficient and effective discharge processes across the Mersey Community Hospital (MCH). The Clinical Nurse Consultant - Discharge Planning is responsible for: * Leading the strategic and operational development and implementation of policies and procedures that maximise patient access. * Providing day to day leadership, management and advice to staff regarding the hospital’s discharge process. * In collaboration with MCH and community health groups, developing effective partnerships to undertake projects related to the continuum of care to achieve best practice in Patient Flow Management. You will need: * The Clinical Nurse Consultant - Discharge Planning works in collaboration and consultation with the Patient Flow Manager, Nurse Unit Managers, Nursing Directors, and respective Group Managers. * Function as a clinical resource for hospital and community staff on issues and processes relating to admission, hospital stays and discharge planning. * Develop and implement training programs related to nursing practices that support efficient and effective discharge planning for nursing, medical and allied health staff whilst maintaining high standards of continuing education and personal professional development.
