MINIMUM QUALIFICATIONS
Education: Graduation from an accredited high school or possession of a high school equivalency certificate.
Experience: One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.

MAIN PURPOSE OF JOB
This Coordinator of Special Programs II, HS position is a part-time (32 hours/week) position and will provide ongoing screening and referral, Care Coordination Services, and collaboration between consumers and community partners such as the Recovery Wellness Center, DUI/Drug Court, Probation/Parole, Coordinated System of Homeless Services, Residential Treatment Providers, Transitional Programs, and the Behavioral Health Treatment Center and other related services to consumers and their families with substance use and mental health issues in Howard County who seek to recover from these disorders. This position also requires grant monitoring experience, which includes ongoing data and progress reporting in the designated electronic health record (EHR) system. This position will provide program grant monitoring of program funds, which may include monitoring contracts to ensure compliance with performance standards, expense limits, and service provisions. This position will also conduct grant monitoring/site visits bi-annually and submit audit documentation to the Behavioral Health Administration (BHA)/Bureau Director by due dates. Additionally this position is designed to assist individuals with substance use as they move from one level of service to another with the goal of providing smoother and stronger connections and support between treatment agencies and community support networks. It requires knowledge of treatment and community services as well as Care Coordination and Case Management Services. Multicultural experience, training, and knowledge regarding how to refer and connect consumers from many populations to services is critical in order to assist in transitions back to communities. This position will also be responsible for conducting outreach to community and behavioral health providers as needed.
