We are offering an exciting opportunity for a motivated and compassionate Community Health Care Assistant to join our Coastal Community Nursing Team in Cornwall.
This role involves delivering high-quality nursing care to patients in their own homes, including those receiving end-of-life care. Duties may include wound care, venepuncture, catheter care, pressure area management, and personal care.
We value integrated, patient-centred care and are committed to supporting your professional development. Previous care experience is essential.
The role requires the ability to travel independently across the Coastal PCN area. Travel time is included in your shift, and mileage is reimbursed at Agenda for Change rates.
This is a community-based position, covering a wide geographical area from 8am–10pm with 7.5-hour shifts.
Please note: I f you require sponsorship, please check the eligibility criteria for a Certificate of Sponsorship on the gov.uk website before applying.
To carry out assigned tasks while regularly working without direct supervision involving direct care to patients and carers to support the registered nurse.
To work as a member of the community nursing team within a given geographical area.
The District Nursing Team work on a flexible rota shift system covering the period from 8am to 10pm which includes an on call system offering a high standard of service.
We’re an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people’s physical and mental health. We also provide specialist support to people with dementia or a learning disability.
We are a people organisation and people matter to us. As part of the team, you’ll help support the health and wellbeing of the people who live and visit this beautiful part of the UK.
Over 4,000 people make up the Trust. This includes doctors, nurses, therapists, plus admin and support staff.
We work in people’s homes, in community clinics and bases. Some staff work from one of our 13 community hospitals. Our aspiration is to have great people, provide great care, be a great place to work and a great partner.
Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increases the numbers of people who use our services.
