Description:
Support the Specialist Nursing and Allied Health Care professionals to provide advice and support to respiratory patients in the community but will also assist the respiratory team at the acute hospital sites to facilitate early discharge onto the respiratory virtual ward. In conjunction with the clinical Nurse Specialist and medical colleagues assist in the education required by patients prior to discharge, including the completion of appropriate bundles. Work with other team members embracing the long-term conditions and virtual ward agenda thus preventing avoidable hospital admissions
