Job Summary:
Optimae LifeServices, Inc. in DeSoto, IA is hiring a full time Team Leader. The supervisory role of Team Leader (TL) is to be an accessible and competent resource for staff who work directly with customers in the Dallas County residential sites. They are role models for the provision of quality services by demonstrating responsible, ethical, customer-centered behaviors.
Education & Work Experience:
· Must be at least 18 years of age.
· A high school diploma or GED is required..
· A minimum of one year of experience serving persons with disabilities or working in the delivery, planning, coordination, or administration of human services is required.
· Be a licensed driver with a reliable, insured vehicle
· Able to use a computer, experience with Microsoft Office, and to communicate effectively in English both verbally and in writing
· Pass a background check including: Child & Dependent Adult Abuse, criminal record, and driving record checks
Knowledge, Abilities, and Skills:
· Ability to foster communication, cooperation, and problem solving between customers, Optimae team members, & other organizations.
· Ability to promote quality assuarance by developing, organizing, and maintaining systems of accountability
· Ability to build morale by developing staff competency through ongoing training and sustained mentoring of staff
· The Ideal candidate will have strong teamwork & leadership skills, strong problem solving abilities, & the ability to teach others.
Duties & Responsibilities:
· Assist in the hiring, training, & disciplinary action of direct support staff members as needed.
· Serve as mentor & coach to direct support staff members, promote teamwork & build moral, & problem solve staff issues
· Coordinate and lead the service delivery and service planning process to ensure provision of services based on customer desires & goals
· Coordinate maintenance of comprehensive customer records in compliance with Medicaid, DHS, CARF, and other applicable standards.
· Coordinate services & procedures to provide for the health and safety of the customers
· Maintain human & civil rights in a manner consistent with Optimae’s mission and core values
· Coordinate all transfers & discharges with customers, including discharge planning, future service planning, & review
· Provide direct service when it is necessary to meet the customers health & safety needs.
· Provide on-call support by carrying a cell phone & responding to calls in a timely manner on a rotating basis
· Work as a team member to achieve established financial objectives.
