Description:

To work as a member of the Multi-disciplinary Team (MDT) across the unit
To plan and facilitate a varied and ongoing activity programme of therapeutic, social, creative and skills based activities for patients on an individual basis and in a group to enhance the quality of life of patients and contribute to their ongoing rehabilitation goals
To work closely with the MDT in identifying and facilitating patient centred activities.
To provide culturally appropriate activities for patients.
To work as a team member to deliver clinical interventions under supervision and direction, using activities as a treatment medium to support patients to achieve/develop/maintain daily living skills.
To support patients to engage in social, educational, vocational, therapeutic and recreational activities.
To plan, implement and review a range of activities using individual and group work approaches, under the supervision and direction of staff on the unit.
To support patients to engage in care plans which have been developed in partnership with the individual.
To support and assist patients with managing their self-care needs as and if identified as part of a care plan.
To assess risk and manage it effectively, in line with trust risk policies.
To provide information regarding the delivery of activities to all patients and other professionals within the team.
To be aware of community resources in order that a socially inclusive focus towards discharge planning is maintained.
To demonstrate effective communication skills and form positive working relationship, using warmth and empathy that enables patient empowerment.
To promote the role of the activity coordinator within the service.
Gather feed back from patients and carers
Come and join the Home1st inpatient intermediate care team at Windsor Intermediate Care Unit , 19 -bedded unit in Bletchley . Our purpose is to provide an enhanced Intermediate Care, rehabilitation and sub-acute service predominately for the older person. Preventing acute hospital admissions and facilitating early discharge are our primary goals. Come and enhance our patients experience by providing activities in groups and one-to-one, as you work along side therapists and nurses.

To provide culturally appropriate activities for patients.

To assist in the assessment process under the supervision of a qualified OT for example; the Model of Human Occupation (MOHO) assessments.
To support patients to engage in social, educational, vocational, therapeutic and recreational activities.
To plan, implement and review a range of activities using individual and group work approaches, under the supervision and direction of staff on the unit.
To support patients to engage in care plans which have been developed in partnership with the individual.
To support and assist patients with managing their self-care needs as and if identified as part of a care plan.
To demonstrate an awareness of and apply OT techniques and activities that are relevant to the patients.
To assess risk and manage it effectively, in line with trust risk policies.
To work in partnership with other agencies, for example signposting to external community resources.
To be aware of and sensitive to the safeguarding needs of children and adults, and knowledgeable both about potential indicators of abuse or neglect and about what to do if they have concerns about a child’s or adult’s welfare.
To provide information regarding the delivery of activities to all patients and other professionals within the team.
To be aware of community resources in order that a socially inclusive focus towards discharge planning is maintained.
To demonstrate effective communication skills and form positive working relationship, using warmth and empathy that enables patient empowerment.
To demonstrate skills and resourcefulness in communication when barriers to understanding are present, including utilising interpreting resources.
To promote the role of the activity coordinator within the service.
To maintain partnership working with the other clinical service settings, and other statutory and non-statutory providers.
To attend and contribute to Activity Coordinator/OT specific meetings/team clinical and business meetings.
To provide feedback, to Activity Coordinator/OT and MDT colleagues, from specialist interest groups and forums.
Gather feed back from patients and carers
To ensure timely, accurate and informative records of clinical contacts are recorded in the patient’s records which meet all standards, according to Trust policy.
To contribute to the recording of patients intervention plans under the supervision of occupational therapists.
To provide care for patients who may become distressed or present with challenging behaviou
