ABOUT THE ROLE:

The Activity Support Partner deliver’s person-centred care and activities to participants at our Waitara hub. You will be implementing participant’s goals to ensure that their activities incorporate what is important for them and what is important to them. You will be providing active support which can include personal care, mobility support and transporting participants to and from their home.
Our main hours are Monday to Friday from 8am to 4:3pm with the potential to work occasionally up to 6pm.

DUTIES:

• Interacting positively with participants, families, carers, and stakeholders.
• Undertaking all required aspects of participant’s personal care.
• Making decisions using sound judgement to support and advocate for the participant.
• Accompanying participants on excursions or outings.
• Assisting the participants to implement their goals.
• Ensuring familiarity with the participant’s profile and always carry their emergency plan.
