Responsibilities:
- Provide timely comprehensive recovery-oriented assessment, care and interventions in collaboration with consumers /carers/families.
- Ensure carers, family and significant others are engaged through our consumer’s journey as appropriate.
- Provision of intensive community-based interventions and outreach support as clinically indicated to consumers and their families
- Work in partnership with internal and external stakeholders.
- Well-developed skills in writing and an ability to promptly prepare on the appropriate documentation for assessments, treatment plans, transition summaries and other forms of documentation.
- Participate in discipline specific clinical supervision and workload management. This will include attendance and participation at clinical review and other reflective spaces.
- Ensure contact hours are completed each month and entered into the appropriate data base.
