Description:

The flagship dementia wards based on Kingsway site Derby are offering a role for a qualified ACP.
The successful Advanced Clinical Practitioner (ACP) will share leadership with other senior clinicians and managers using specialist knowledge and skills to provide healthcare autonomously to patients within the older adult inpatient services. The post holder will work as part of the senior leadership team, they will practice autonomously , whilst being accountable and self-directed in line with their Professional Code of Conduct.
The post holder will be a registered nurse or AHP and hold an MSc in Advanced Clinical Practice or equivalent at MSc level together with a portfolio that meets the required standard of the Derbyshire ACP panel. They will be an independent Non-Medical Prescriber registered with the NMC or HCPC.
The ACP role encompasses four pillars of delivery. These are (1) Clinical (2) Education (3) Management/ Leadership (4) Research/Audit/Quality Improvement. Each pillar will have protected time.
They will practice independently at an advanced level demonstrating in depth knowledge and competence in all aspects of medicine, encompassing advanced clinical assessment, critical thinking and clinical management skills that are evidence based and deliver high quality patient centred care.

All Advanced Clinical Practitioners (ACPs) should be competent to practice all core role competencies and, as such, be capable of working across all clinical areas to the standards established within the Derbyshire Advanced Practice Network. However, there will be variations in the level of skill above this standard, depending on the specialist area of practice. ACPs are expected to support the establishment and delivery of high-quality, evidence-based care.

  • Independently conduct advanced practitioner-level holistic assessments, planning, implementation, and evaluation of complex mental and physical healthcare needs for working-age adults and older adults.
  • Undertake physical and mental health examinations as indicated by the individual’s presenting condition; autonomously analyse complex clinical signs and investigation results to consider differential diagnoses, formulate decisions, and diagnose conditions, initiating treatment as required – deciding when necessary to refer to senior medical colleagues.

As a ward-based ACP, you will be on the ward for four days each week, with one non-clinical day to support development, leadership, and research.
You will work within our flagship dementia specialist ward alongside the consultant, staff grade doctor, and supporting junior doctors.
You will provide clinical leadership to the ward team and be a valued member of the medical team
