Description:
The postholder will require a Masters degree in Advanced Clinical Practice. This position will be a varied role, with management responsibilities for the team, clinical leadership, clinical development of the team and clinical reviews with Care Home residents. In terms of management responsibilities, the postholder will be responsible for the day-to-day management of the clinical team including a trainee ACP, Specialist Frailty Nurses, and Nursing Associate, with support from Advanced Pharmacist Practitioner, GP Clinical lead for the service and the Human Resources Manager. This will include 1:1s, team meetings, any new starter processes, probationary reviews, and carrying out annual appraisal for existing clinicians.
The postholder will also work closely with the GP Clinical Lead and Primary Care Network (PCN) leads to monitor delivery against the requirements of the EHCH specification and identify opportunities for improvement. The clinical role will require the postholder to have extensive experience in assessing the needs of older people, including experience of the holistic comprehensive geriatric assessment and be able to work independently within the Care Home environment. The postholder will require advanced communication skills to discuss and complete proactive individualised clinical management plans for Care Home residents. The desired outcomes from assessments include improving the health experience of Care Home residents, having a positive impact on primary care service provision, improving interprofessional relationships, and reducing unnecessary and potentially harmful hospital admissions.
The postholder will communicate individual management plans with system partners and develop relationships with the PCN GP practices, Care Homes, community teams, mental health services, palliative care services, and social services, to enable residents to meet their health goals and be cared for in their preferred place of care. The postholder will need to be confident with Multidisciplinary working style and utilising the current Multidisciplinary Team processes in place to appropriately discuss a residents care. The role involves clinical discussions with other members of the team and taking responsibility for the decision-making during these discussions. The EHCH GP Clinical Lead is available for senior support at designated times throughout the working week.
The postholder will be expected to bring their wealth of experience to enhance team working and ensure the team continues to grow. The postholder will understand the need to work closely with PCN Leads to ensure plans are in place to meet demand within the set KPIs within the PCN contract
