Description:

This exciting new post will support the management and leadership of the Nurse Led Acute Frailty Response (AFR) Service to ensure appropriate assessment and intervention is carried out to enable patients care to be transferred safely back to community teams and away from acute services. The post holder will also provide frailty support across a range of primary care, community and acute settings.
The post holder will be expected to act as a practice leader, managing their own workload and work across professional, organisational,
agency and system boundaries to improve services and develop practice. They will provide leadership, support the teams across community
frailty services and acute frailty response teams (AFR), and demonstrate clear principles of teaching and learning, as well as supporting others
through teaching, supervision and assessment.
The post holder will be an experienced advanced level practitioner with, or working towards, IP and possess autonomous expert skills in assessment, examination with the ability to provide a diagnosis and management plan for patients across all settings working with a range of stakeholders.

As an advanced, autonomous practitioner the post holder will provide patient-centred care, encompassing expert skills in assessment,
examination, identifying differential diagnosis for the treatment of patients utilising our services.
Priorities of the post holder will be on the provision of safe, effective and efficient care. The post holder is expected to work as a highly skilled senior practitioner with expert knowledge, delivering evidence-based practice to a designated population, supporting relevant policies and strategies such as the redesigning services for older and/or frail people as part of the Health Board’s response to the Six Goals for Urgent and Emergency Care.
In accordance with the Advanced Clinical Practice framework (National Leadership and Innovation Agency for Healthcare, 2010) the post
holder will align duties and responsibilities to the pillars of advanced practice.
The post holder practices advanced nursing within the Framework for Advanced Practice in Wales 2010. Core service delivery is covered by
four pillars of practice, Clinical Practice, Management and Leadership, Education and Research, and will be expected to monitor standards of
nursing care, patient experience and patient safety and take appropriate action to maintain and improve them.

The ratio of allocated time will be up to 80% clinical practice with the remaining time appropriately planned to ensure all of the required pillars of Advanced Nurse practice are met.
Provide support and in reach to ABUHB care home and acute settings as required.
Promote and support effective transfers and discharge across care settings and home.
Undertake comprehensive nursing assessments, which may include physical examinations and comprehensive patient histories.
Will support the redesign of older people workstreams and line manage Band 6s within the team.
Demonstrates leadership and innovation in work contexts that are unfamiliar, complex and unpredictable and that require solving problems
involving many interacting factors.
Reviewing strategic impact/outcome of the work of the team.
Create and promote a learning environment.
Understand the principles of teaching and learning and actively engage with educational providers.
Active involvement in research and audit to critically appraise and evaluate care.
Use advanced clinical assessment skills to autonomously manage patients to ensure that optimal physical and psychological care
needs are met.
Will support the redesigning services for older and/or frail people programme and subsequent work streams and projects, and have overall
responsibility for supervising and leading the nurse practitioner team.
To balance conflicting demands on time and choosing between strategic work priorities.
Manage and prioritise the diverse and challenging workload within a multidisciplinary organisation.
There will be a requirement to deal with conflict and confrontational professionals reluctant to acknowledge and embrace the national strategy and standards required for the provision of safe and effective care to patients.
Maintain an awareness of developments, specialist expertise knowledge within a rapidly moving agenda to enable the provision of highly specialised expert advice based on interpretation of robust reliable evidence.
Respond to demands for advice, reports and briefs with short time frames.
