In the heart of Buckinghamshire countryside the Trust is located conveniently close to London and Oxford. We provide services to three main hospital sites, Wycombe, Amersham and Stoke Mandeville and some local GP practices. As a department we are ambitious and continually look to be at the forefront of innovation in Pharmacy careers. We are committed to the development of our staff.
We currently see 2-3000 patients a year in this pharmacist led Anticoagulant Clinic initiating and reviewing oral anticoagulation with the DOACs. In addition we run warfarin dosing clinics in 3 GP practices in South Bucks.
We are looking for an enthusiastic, dynamic and motivated Pharmacist to join our pharmacy led prescribing team working in clinics both in the acute Trust and in GP practices to optimise prescribing of anticoagulants and assisting with training to both primary and secondary care providers.
Main duties of the job
You will be responsible for prescribing anticoagulants in one of the many clinics which are run by Trust. Patients are referred into our services via primary care or the acute Trust Clinicians. Excellent communication and patient consultation skills will be required to engage and support GP’s, consultants, non-medical practitioners and patients and to ensure appropriate prescribing and patient follow up takes place. You will have a post graduate qualification in Clinical Pharmacy or extensive clinical experience. An independent prescribing qualification will be required or the commitment to work towards one. Previous experience of anticoagulation will be of benefit but not be essential as training will be provided. Experience of medicines optimisation and prescribing in at least one long term condition would be an advantage. Clinics run Monday to Friday however the opportunity may be available to join the Pharmacy on call and Weekend Teams.
